The minimum information required to create a payee record is just the name of the person.

Last name and first names

Both the last name and first names of the payee should be entered. They are used by the system wherever you need to select a particular person.

Sex, birth date and telephone number

These are not required, and are provided for your own reference should you need them.

Address

The address will be printed on the payee's payment summary at the end of the tax year.
